This is about making with your hands ,added with some tea and listing :) So, make yourself 6 envelopes, size and look is up to you and material too, but try to make envies that your partner would like, you can use newspaper, advertizes, painted paper or whatever you find interesting and pleasant.

Then check what flavours your partner likes, and send 5 bags of tea.

And finally, like the name of this swap says 6+ 5 is 11, write a note to your partner and list 11 things you like or love.
