Create one 4x6" rolodex card for your partner. This is artist's choice. Lightly decorate the back of the card and include your name, your SB name and the name of the swap.

I think back in the day when this group was very active, some folks had a preference as to whether the back of the card had the design/information upside down or downside up - so you can read it without pulling the card out. If you have a strong preference about this, please add this information to your profile so all your future partners will know. Or you can simply message your partners each time to let them know.
