I saw your rating for my Valentine's Dotee and replied to the rating instead of posting here! lol That particular box was a die cut box available in packs ages ago from Stampin Up. BUT you can find very similar printable templates here http://www.ruthannzaroff.com/mirkwooddesigns/templates.htm
There are other "pillow box" patterns around the web in various shapes and sizes that make similar boxes.
I'm glad you liked her though! I had fun making her and can see how addictive they are! Now I just want to make tons!
